We can only accept an address change from the record title owner. Please complete all sections of the address change form.
Form 1099 Inquiries
Superior Midstream’s IRS Forms 1099 are mailed on January 31st of each year. Please note, we are not required to furnish this form to foreign owners, incorporated owners, and/or owners with royalty payments less than $10 or annual working interest payments less than $600. We will be happy to assist you with any Form 1099 issues that may concern you. Please contact our Division Order team by submitting the 1099 Inquiry form.
Automated Payments of Revenue
Superior Midstream has begun using automated payments of revenue by ACH (Automated Clearing House). ACH is a payment that is deposited directly into the bank account specified by you, which allows for a more expedited form of payment.
If you would like to receive your revenue payment through ACH, please complete the form. This form can be returned via fax, mail, or scanned and e-mailed to the address provided on the form. It is important to note that if you choose payment by ACH, the payment detail will be emailed to the email address you will provide on the enclosed form. Additionally, please attach a VOIDED check of your checking/savings; or documentation with account and bank routing number.
We will be processing ACH requests in a timely manner as they are received. Please allow appropriate time for all updates to be accomplished.
